New Child Enrollment
A completed enrollment application packet, including a medical assessment from the child’s pediatrician is required in order to place a child in a Bell Center program. To download the application, click on the links below. Please note that the Medical Assessment portion of the application must be completed by your pediatrician. A $25 fee must be submitted with the application packet.
Once the application is received, the Program Director will contact the family to schedule a Family Routine Based Interview, as well as the Child’s Individual Evaluation. When completed, a child can begin receiving early intervention services at The Bell Center.
