How to Enroll a Child
A completed enrollment application packet including a medical assessment from the child’s pediatrician is required in order to place a child in a Bell Center program. To download the application, click on the links below. Please note that the Medical Assessment portion of the application must be completed by your pediatrician. A $25 fee must be submitted with the application packet.
